Which field flattener are you using?

You only need the adapter that came with your refractor and add the correct spacers behind. If your camera's focal flange distance (FFD) is 18mm (ZWO and QHY are usually both 17.5mm), then you substract this lenght to the 55mm (55 - 18 = 37mm) so you need to add 37mm of spacers. This can include an off-axis guider, a filter wheel or simply spacers as long as the total lenght including your camera FFD doesn't exceed 55mm. If you are using a DSLR or mirrorless, then the T-ring count also as it is external.

Measure 55mm from back of the reducer to the camera sensor.

On my camera you need 55mm - 17.5mm = 37.5mm in 42mm/48mm diam spacers (or less if you have a filter holder, usually 21mm).
